2012-2013 FHC Spartan Hockey Apparel Order Form

 

JERSEY DEPOSITS/PURCHASE


Jerseys will be distributed during the first week of practice. A $175.00 deposit is required for all jerseys issued. If you are issued multiple jerseys, we require multiple checks. They will be returned to you at the end of the season after the jerseys are returned in good condition and all financial obligations to the Club have been paid.



Players wishing to purchase their jerseys for a keepsake or to retain the jersey number for future years may do so for (TBD).  This will include the home and away jerseys.  Care and maintenance will become the sole responsibility of the player.  A garment bag will not be required but is recommended to maximize the life of the jerseys.  Purchase of the jerseys in no way guarantees team placement for future years.



Swing Player Jerseys

Players will need to leave a deposit check for BOTH sets of jerseys.  Varsity swing players that want to purchase their Varsity jerseys can purchase their Varsity jerseys and leave a deposit for the JV jerseys.

WARMUP JACKET/PANTS


The Francis Howell Central Hockey Club Coaching Staff has required that ALL players wear the Club approved warm-up to games/practices.  This will be the case once again for the 2012/2013 season.  This includes all three FHC teams.



For those needing a wind suit, please go to Johnny Macs thru August 31, 2012 to be sized. Please identify yourself as an FHC hockey player. They will have a list of players. No payment is due until they are delivered and payment will be paid to the club. Jackets are 58.00 and pants are 30.00. Please let John Dickens know what has been ordered via email. To help him keep track of ordering if you did NOT order anything please let him know that as well.
